Office-GAP Project - Adesuwa Olomu, M.D.
With funding from Agency for Healthcare Research and Quality (AHRQ), Dr. Olomu and her co-investigators are testing the implementation of an outpatient program “The Office Guidelines Applied in Practice" (Office-GAP) in Federally Qualified Health Care Centers (FQHCs) in Michigan. The Office-GAP Program is designed to improve care and health outcomes for cardiovascular and diabetic patients in FQHCs.
Office-GAP includes: 1) Patient activation intervention; 2) Physician training for patient activation intervention; and 3) Decision support/Checklist intervention (DSI) used in real time in the office. Office-GAP aims to produce high quality shared decision making (SDM), increase uptake of guideline-recommended care, and ultimately improve patient outcomes for populations with high risk for cardiovascular disease in Federally Qualified Health Care Centers. It only requires a one time group visit and 2 follow up visits with your doctor. You will be paid a total of $30 for attending the group visit and the 2 follow up visits.
Patients at Ingham County St. Lawrence Clinic (Saginaw & MLK) & Ingham County Adult Center (Cedar St) are eligible to participate in this study.
